I acknowledge that I have been fully informed of the potential risks associated with getting a tattoo. I understand that these risks, both known and unknown, may result in injury, including but not limited to infection, scarring, difficulty in detecting melanoma, allergic reactions to tattoo pigment, latex gloves, or soap. Despite being aware of these risks, I voluntarily choose to proceed with the tattoo application and fully accept all associated risks.*

The artist has provided me with the opportunity to ask any questions regarding the tattoo application, and all of my questions have been answered to my complete satisfaction.*

Tattoo inks, dyes, and pigments are classified as cosmetics and are not approved by the U.S. Food and Drug Administration (FDA).*

The artist and tattoo studio have provided me with aftercare instructions for my tattoo, and I understand them. I agree to follow these instructions during the healing process. I acknowledge that failure to follow the aftercare instructions may lead to infection. If any touch-up work is needed due to my negligence, I understand that I will be responsible for the cost.*

I confirm that I am not pregnant and am not currently breastfeeding.

While there are no official guidelines regarding tattooing during pregnancy, it is generally not recommended by dermatologists, obstetricians, gynecologists, or tattoo artists due to potential risks.*

While there are no specific guidelines around getting a tattoo while pregnant, dermatologists, ob-gyns, and tattoo artists don’t recommend it.*

I confirm that I am not under the influence of alcohol or drugs and that I am voluntarily choosing to receive a tattoo from the artist without any form of pressure or coercion.*

I confirm that I have no medical conditions, including diabetes, epilepsy, hemophilia, heart conditions, or any other issues that could affect the tattoo process or healing. I am not on blood-thinning medication. If I’ve had an organ or bone marrow transplant, I’ve consulted my doctor and followed all required preventive measures. I also confirm that I have full mental capacity to make this decision.*

The tattoo, as described or presented to me, has been accurately drawn according to my specifications. I understand that the artist is not responsible for the meaning, spelling, or accuracy of any symbols or text that I have provided or selected in consultation with the artist.*

I acknowledge that variations in color, shading, and design may occur between the tattoo artwork I have selected and the final tattoo once applied to my body. I also understand that, over time, exposure to sunlight and the natural dispersion of pigment under the skin may cause the colors and clarity of my tattoo to fade.*

I understand that a tattoo is a permanent alteration to my appearance and can only be removed through laser or surgical procedures, which may be costly, disfiguring, and unlikely to fully restore my skin to its original condition prior to the tattoo.

I hereby release all rights to any photographs taken of me and my tattoo, granting consent for their reproduction in print or electronic form. (If you are not comfortable with this, please inform us, and we will ensure your photo session remains private.)*

I agree to reimburse the artist for any attorneys' fees and costs incurred in any legal action I may bring against the artist in which the artist is the prevailing party. I further agree that any disputes arising out of or related to this agreement shall be governed by the laws of the State of California, County of Los Angeles, USA, and that the courts in that jurisdiction shall have exclusive personal jurisdiction and venue.*

If any provision, section, subsection, clause, or phrase of this release is found to be unenforceable or invalid, that provision will be severed from the contract, and the remainder will remain in full effect as though the unenforceable portion had never been included.*

I acknowledge that I have been given sufficient time to read and understand this document, and that I have been advised to consult legal counsel if I have any questions about its implications. I further confirm that this document was not presented to me at the last minute, and I understand that by signing, I am entering into a legal contract that waives certain rights to seek recovery against the artist.*

Based on the foregoing, I hereby waive and release the artist, to the fullest extent permitted by law, from any and all liability for any claims or causes of action, including personal injury or other damages, whether direct or consequential, that may arise from or relate to the application of my tattoo by the artist, on behalf of myself, my estate, heirs, executors, or assigns.
